For all appointments & inquiries, please call (954) 917 4941 during normal business hours or click here to request an appointment online.

Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointments, please remember to bring the following

  • New Patient Forms
  • Insurance Card(s) and Picture ID
  • Referral if your insurance requires one
  • Any and all recent chest X-rays, CT Scans, PET Scans and prior sleep study reports
  • Discs and Films are not required, but may bring along if you have them

Cancellation/ No show Fee

There is a cancellation/no show fee of $20.00 that will be charged if you do not notify
the office 24 hours in advance, of any change in your appointment.

Driving Direction

Directions from I-95 North (Broward / Palm Beach):

Take I-95 to Sample road. Exit Sample road west. Travel approx. 4 miles to STATE ROAD 7/US441. At STATE ROAD 7 make a left (south). Our office is a quarter of a mile down on the right side (west side of STATE ROAD 7) in the Outpatient Pavilion, attached to Northwest Medical Center.

Directions from I-95 South (South Broward / Miami-Dade):

Take I-95 north to Copans Road. Exit Copans Road and go west. Travel approx. 4 miles to STATE ROAD 7/US441. At STATE ROAD 7 make a right (north). Our office is approximately one mile north on the left side (west side of STATE ROAD 7) in the Outpatient Pavilion, attached to Northwest Medical Center.

Directions from Florida Turnpike North (Broward / Palm Beach):

Take the Florida turnpike south to Sample road. Exit Sample road west. Travel approx. 2 miles to STATE

ROAD 7/US441. At STATE ROAD 7 make a left (south). Our office is a quarter of a mile down on the right side (west side of STATE ROAD 7) in the Outpatient Pavilion, attached to Northwest Medical Center.

Directions from Florida Turnpike South (South Broward / Miami-Dade):
